Kentucky’s vaccine requirements for school entry, which is modeled after the Centers for Disease Control and Prevention, include the following:

Students entering kindergarten must have a physical exam, a dental exam, an eye exam, and completed age appropriate immunizations. To see a list of immunization schedules please visit the Washington County Schools website under the parent tab or contact your pediatrician for more information.

Students going into sixth grade must have their immunizations updated and a physical exam. The physical form required by the state is different than the one required for sports physicals. Please use this Student Physical Exam Form.

All students who are 16 years or older are also required to have two doses of the Meningococcal ACWY vaccine. If the first dose of Meningococcal ACWY was received at age 16 or older, the second dose is not required for school entry.

Families can schedule an appointment for a physical or immunizations with their pediatrician’s office or at the Healthy Kids Clinic in Washington County Schools. To make an appointment through the Healthy Kids Clinic, you will need a current registration form on file with the school nurse.

The Centers for Disease Control and Prevention (CDC) and Kentucky Department for Public Health (KDPH) recognize immunization as one of the most effective tools in preventing disease and reducing the risks associated with exposure to certain diseases. KRS 214.036 requires parents who object to immunization of their child to provide a written sworn statement objecting to immunization of the child on religious grounds. This Immunization Exemption Form will need to be completed and returned to your child's school.

All vaccines must be administered at the correct intervals for your child to be considered compliant. If your child received any vaccines that were not administered at the correct intervals, an additional dose of that vaccine may be required. You will be contacted by the school nurse if your child is noncompliant on any vaccines.

Important Notice for Parents of All Students Entering 6th Grade

Medications

No medication will be dispensed at school unless the following criteria are met.

1.    Medication in its original labeled container.

2.    Authorization forms must be completed and returned to the nurse before medication will be administered at school. Authorization forms are available at each school from the nurse.

 Medication must be brought to school by an adult, not the student. If a medication will be needed for both home and school use the parent should request that the pharmacist provide them with an extra labeled medication bottle. Medication not picked up by a parent, within 2 weeks of its discontinuation of use or the end of the school year, will be disposed of by the school nurse or designee.

**This includes both prescription and non-prescription medications. Non-prescription or over-the-counter medications include but are not limited to acetaminophen, ibuprofen, cough drops, topical creams, and sunscreen.

Several over-the-counter medications are available at school as part of the Healthy Kids Clinic and can be administered if a completed and current Healthy Kids Clinic consent/registration form is on file.
